PARTICIPANT RIGHTS
We want to be sure you are aware of your rights as a research study participant , and that you know what to expect if you participate. Participation in this research study is voluntary, and if you choose not to participate, you will not incur any penalties. We do not anticipate any risks to you from participating, but the information gained from this study will likely not benefit you personally. Any information you share with us will be kept confidential. Any identifying information (such as your name, address, phone number) will be kept separate from any study information. Because this is a long-term study, it is important for us to be able to stay in contact you for the duration of the study. Therefore, we will ask you to provide some information that will help us find you in case you move or change your phone number (for example, names of other people who might know where you have moved). None of the study information will have your name attached. It will only have a study ID and will be kept in locked filing cabinets or password-protected electronic databases behind institutional firewalls. No information will be presented or shared in a way that will identify you personally.

Welcome to the 10,000 Families Study! We are excited that you are interested in our research project. The 10,000 Families Study is a University of Minnesota study about family health. We want to understand how lifestyle, environment, and genetics affect health over time. We want to include families from all communities across Minnesota!
This survey is to find out if you and your family qualify for the study. If you do, we will send you an email with information about the study and how to participate.
All families are welcome!
Participation in the 10,000 Families Study includes: First step: Invite other family members of all ages to participate Second step: Complete a health questionnaire for yourself and for any children (under age 18) you enroll Third step: Take part in a family health visit and return simple at-home tests Follow-up: Provide updates on your health about every two years
As a thank-you from the study, you will receive: some results from your health visit and your home radon test, $30 e-gift card (awarded after you complete your health visit: $15 for completing the online health questionnaire+$15 for the health visit), $5 e-gift card for returning your silicone wristband, $5 e-gift card for returning your radon test kit (1 per household), and $15 e-gift card for completing the follow-up questionnaire Participation in the 10,000 Families Study is voluntary, and all your information is kept confidential and protected.

And to sweeten the deal, after giving the blood sample you will receive an additional give-away of food or ride tickets to enjoy at the Fair.
