Thank you for your interest in taking this LyRA study survey and for your support of our research. Please review the qualifications on the LyRA website page prior to filling out this survey. We ask for your patience as we move through the randomized list of eligible dogs which could take several months.
We need to ask the questions below to make sure your dog fits the criteria for the study and to ensure we get the samples we need. Completing all of the questions is crucial to evaluate your dog's eligibility for the study. Incomplete surveys cannot be fully evaluated.
