Submission of this form along with the required matireals will facilitate the review of previously completed graduate level coursework for equivalency to Doctor of Nursing Practice Requirements.

Petitions based upon prior coursework must inlcude the follolwing:

1. Unofficial trianscript with final grade(s) for the completed course(s)

2. Syllabus from the completed course(s)

Petitions based upon professional experiece must include teh following:

1. Resume/CV

2. Supporting material that demonstrates competency that aligns with the objectives of the course
