Portfolio Plan: Narrative and Data

All Medical School PIs and research groups conducting biomedical research in IRB Tiers 2-4 must submit a Portfolio Plan using this form to sunrise their biomedical research and assure they are mitigating risk. Documentation of Medical School department head or OACA Center/Institute director approval is required prior to submission.

Please submit the Narrative Form as a Word document using the following naming format: Department or Center_PI Last Name or Research Group Name_Date of Submission_Narrative Form.

Please submit the Data Form as an Excel document using the following naming format: Department or Center_PI Last Name or Research Group Name_Date of Submission_Data Form.
