D2D People Helping People: Betting Understanding of Sarcoma
You have been invited to participate in the research study listed above. Please read this form and ask any questions before agreeing to participate. This study is being conducted by Kristine M. Nachbor, M.D. Candidate, 2022, University of Minnesota, Dr. Kathryn Dusenbery, M.D., University of Minnesota, and the Rein in Sarcoma Foundation.
Procedures
If you agree to participate, we will ask you to do the following:
1) Confirm with the research volunteers on site that you are of the age of 18 or greater, you are a current resident of Minnesota, you are able to independently complete the electronic questionnaire, you are not currently under the influence of alcohol or drugs, and you are not currently a member / affiliate of KWRIS, a current or past Sarcoma Scholar, or a research volunteer for this study.
2) Complete an electronic questionnaire composed of 21 questions.
Confidentiality
The records of this study will be kept private. Published reports will not include any information that will make it possible to identify participants.
Voluntary Nature
The questionnaire is optional and may be terminated at any time if the participant so wishes. Your decision to participate, decline, or withdraw from this study at any time will not affect current or future relations with the University of Minnesota. Please request a paper copy of this consent form, if interested.
Risks and Benefits of Participation
This study involves completion of an electronic questionnaire and will not involve collection of personal health information or identifiers. Completion of the questionnaire will not directly benefit participants; however, educational materials provided to participants following completion of the questionnaire will be of educational value.
Compensation / Incentives
Each participant will be given an individually wrapped Lifesaver candy as incentives for participation. Each participant will also be provided with educational materials. Acceptance of these incentives is optional.
